C♯6 (" Spank Thru" Numerous Live Versions (1987-1989) & Sub-Pop 200 version) I think these notes should be marked as blue. It is very clearly falsetto.

If anything, the note could be green, but as we all know Kurt’s one dying wish was to never have green notes so that will not happen.

I think Kurt's scream at 0:31 actually goes higher than C#6. D6 or Eb6 is what it seems to be.
Edit: Electric Miami is correct that the pitch is mostly a sharp C#6. If the note ever reaches D6 it's probably not countable, but you be the judge of that.

Speaking of weird high screams, here is another interesting one. It starts off too distorted for me to discern a pitch but breaks into somewhere around a C#6. Just before the break I think I hear a D6 under the distortion, but I’m not sure. I guess I’ll also use this as an excuse to show some other interesting stuff not on the thread. If we could hear it more clearly, I'm sure it would sound a lot softer.

I added the "If You Must" live scream as a B5 because he's more cracking into D6 rather than really controlling any pitch higher than B5 in this bit. I also added the E♭2 and other notes from that radio session you linked, however the G♯5's sound like they're done by someone else to me

Also I think the D6 clip is actually a crowd member screaming. It's faint but kurt still seems to be singing when the scream starts and it sounds as if it occurred right next to the tape.
The big Cheese scream i'm vibe typing as E5 or Eb5, leaning towards the latter atm.

Kurt sings some of TAD's songs. "High on the Hog" has D3's and G4's throughout. "Sex God Missy" has G4's and a few of what I think are E5 screams at 3:53, 4:35, and 5:54. "Wood Goblins has G#4's and an A4 at 9:21. "Ritual Device" is almost inaudible, but I heard an A4 and F#4 at a couple points like at 14:19
I think Kurt sings between F#4-G#4 for most of the song, but I think he might touch the 6th octave at 0:36. It's countability is questionable tho.

I've stumbled across some more nonsense Multiple G5s {G5} E5 {E5} D5 I think {D5} Polly sung an octave up to some solid C5s {C5} A D3 I think {D3} This might be a B2. It's a pretty long one for him if it is. {B2} B♭2 {B♭2} G♯2, There's an implied E2 but it might just be air. {G♯2} This whole song is filled with screechy overtoney vocals recorded on the worst piece of recording technology possible. 1:43 struck out to me the most. There's also a normal A4 at 0:55. {Overtoney Goofiness} More overtoney goofiness recorded on a tin can {Overtoney} {Goofiness}

I'm pretty sure that backing E2 in "Spank Thru" is Jack Endino. {He talks about doing backing vocals on the song from 1:17-1:30 here:} {also it kind of sounds like it's less fryish, like it's more natural talk-singing, at mid/lower 2nd octave pitches where Kurt seemed to use fryish techniques}
